Goa Surpasses 1 Lakh AI-Reviewed Chest X-Rays in Public Health System, Detects 18 Lung Cancer Cases

Goa has become the first Indian state to cross the milestone of 1 lakh chest X-rays reviewed using artificial intelligence in its public health system. The AI-powered tool, deployed across government hospitals and primary care centres, has identified 18 previously undetected cases of lung cancer, along with thousands of other abnormalities including tuberculosis, pneumonia, and COPDโ€”demonstrating the transformative potential of AI in large-scale screening and early detection.

The milestone was announced on January 27, 2026, by the Goa Health Department. The AI system, integrated with the stateโ€™s digital X-ray network, automatically analyses images for critical findings and flags high-risk cases for radiologist review. Over the past 18 months, it has screened more than 1 lakh X-rays, achieving high sensitivity for lung cancer and TB while reducing reporting delays from days to hours. The initiative has strengthened Goaโ€™s TB elimination efforts and improved early cancer detection in a state with rising lifestyle-related diseases.

Goa has achieved a significant public health milestone by becoming the first state in India to review over 1 lakh chest X-rays using artificial intelligence within its government healthcare system. The announcement, made on January 27, 2026, by the Directorate of Health Services, highlights the impact of the stateโ€™s AI-based radiology screening programme, which has been operational across district hospitals, community health centres, and primary health centres since mid-2024.

The AI tool, integrated directly into the stateโ€™s digital X-ray workflow, automatically analyses images for abnormalities such as tuberculosis, pneumonia, lung nodules, masses, pleural effusion, cardiomegaly, and other thoracic pathologies. Suspicious findings are flagged for priority review by radiologists, enabling rapid confirmation and follow-up. In the process, the system has detected 18 previously undiagnosed cases of lung cancer many at early stages along with thousands of other treatable conditions, including active TB cases that might otherwise have been missed or delayed.

The programmeโ€™s success stems from its seamless integration with existing infrastructure: X-ray images are captured digitally at public facilities, uploaded to a secure cloud platform, and processed by the AI within minutes. Radiologists receive prioritised worklists with highlighted abnormalities and confidence scores, allowing them to focus on complex cases while routine normals are cleared quickly. This has reduced average reporting time from 2โ€“3 days to under 4 hours for most images, improving patient flow and timely treatment initiation.

Dr. Vishal Patil, Director of Health Services, Goa, described the impact: โ€œCrossing 1 lakh AI-reviewed chest X-rays is a proud moment for Goa. More importantly, it has helped us catch silent diseases early especially lung cancer and TB when treatment is most effective. This technology is proving invaluable in our fight against preventable mortality.โ€

The initiative aligns with national priorities under the National TB Elimination Programme (NTEP) and the National Programme for Prevention and Control of Non-Communicable Diseases (NPCDCS). Goa has been an early adopter of ABDM, with high ABHA linkage rates, enabling AI-flagged cases to be seamlessly followed up with digital records and specialist referrals.

The state government has committed to further expansion, including AI integration for other imaging modalities (e.g., mammography, abdominal ultrasound) and deeper coverage in remote tribal areas. The programme has also generated valuable real-world evidence on AI performance in Indian populations, with ongoing studies evaluating its impact on TB case detection rates and lung cancer survival outcomes.
