Ayushman Bharat Digital Mission India’s Push for a Unified Digital Health Ecosystem

The Ayushman Bharat Digital Mission (ABDM), launched in 2021 by the Government of India, continues to be the cornerstone of the country’s digital health transformation. As of January 2026, the mission has achieved over 420 million Ayushman Bharat Health Accounts (ABHA), linked more than 182,000 Ayushman Arogya Mandirs, and enabled widespread adoption of digital health IDs, electronic health records, and interoperable services across public and private facilities.

ABDM aims to create a secure, federated digital health infrastructure that gives every citizen a unique ABHA ID, allows seamless sharing of health records with explicit consent, and supports telemedicine, e-prescriptions, lab reports, and hospital services on a single platform. Key achievements include high adoption in states like Bihar (91% digital OPD), Andhra Pradesh, and Uttar Pradesh, along with integration of tools such as Scan & Share for paperless OPD registration. The mission now focuses on deepening rural penetration, improving data quality, strengthening privacy safeguards, and enabling AI-driven use cases while addressing the digital divide.

The Ayushman Bharat Digital Mission (ABDM) remains India’s flagship initiative to build a nationally integrated, patient-centric digital health ecosystem. Launched in September 2021 under the Ministry of Health and Family Welfare, ABDM seeks to eliminate fragmented health records, reduce duplication of tests and procedures, and empower citizens with portable, consent-based access to their health information across public and private providers.

As of January 2026, the mission has crossed several major milestones: More than 420 million Ayushman Bharat Health Accounts (ABHA) have been created, giving individuals a unique 14-digit digital health ID. Over 182,000 Ayushman Arogya Mandirs (wellness centres) are now digitally enabled, offering services such as online appointments, e-prescriptions, and digital health records. Scan & Share QR-based OPD registration has been widely adopted in states such as Bihar (over 91% digital OPD), Andhra Pradesh, and parts of Uttar Pradesh, drastically reducing wait times and paperwork. Thousands of hospitals, diagnostic labs, and pharmacies are now ABDM-compliant, enabling secure sharing of lab reports, discharge summaries, prescriptions, and imaging results.

The core building blocks of ABDM include the ABHA ID, Health Facility Registry (HFR), Health Professional Registry (HPR), Health Information Exchange (HIE), and consent managers. These components ensure that health data remains federated (not centralised), patient-controlled, and interoperable across systems addressing long-standing issues of data silos and lack of continuity of care.

Recent focus areas include:

Deepening rural and semi-urban adoption through community-level ABHA creation drives and vernacular-language interfaces.

Strengthening privacy and security with granular consent mechanisms and audit trails.

Enabling AI and analytics use cases (with strict de-identification) for population health insights, disease surveillance, and predictive care.

Integrating ABDM with state-specific digital health projects and private sector platforms.

Despite strong progress, challenges persist. Rural connectivity gaps, low digital literacy among elderly and marginalised groups, varying levels of adoption across states, and concerns over data privacy continue to limit full realisation of the mission’s potential. States like Bihar and Andhra Pradesh have achieved high digital OPD penetration, while others lag due to infrastructure and awareness barriers.

Government officials and industry leaders view 2026 as a pivotal year for ABDM consolidation shifting from enrolment and infrastructure build-out to meaningful usage, clinical impact, and measurable improvements in health outcomes and system efficiency.

The mission remains central to India’s ambition of achieving universal health coverage, reducing out-of-pocket expenditure, and building a resilient, data-driven healthcare system capable of serving 1.4 billion people.
