Telangana to Issue Digital Health Cards as Government Compiles Records of 60 Lakh Citizens

The Telangana government is set to roll out personalized digital health cards to all residents as part of its ongoing compilation of health data for over 60 lakh citizens. The initiative, aligned with the Ayushman Bharat Digital Mission (ABDM), will create secure, portable digital health records accessible via ABHA-linked cards, enabling seamless access to medical history, prescriptions, lab reports, vaccinations, and treatment details across public and private facilities.

The state has already digitized health records for more than 60 lakh citizens through integrated surveys, hospital visits, and primary health centre data collection. The digital health cards will serve as a single, verified identity for healthcare services, supporting real-time updates, teleconsultations, e-prescriptions, and continuity of care. The project emphasizes data privacy under the DPDP Act, interoperability with ABDM, and inclusion of vulnerable populations, with phased issuance starting in urban centres and expanding statewide.

The Telangana government has announced plans to issue personalized digital health cards to every resident as it nears completion of compiling comprehensive health records for over 60 lakh citizens. The initiative, spearheaded by the Department of Health, Medical & Family Welfare in alignment with the Ayushman Bharat Digital Mission (ABDM), represents one of the most extensive state-level efforts to create a unified digital health identity system in India. Health officials confirmed that data aggregation from primary health centres, government hospitals, private facilities, vaccination drives, and door-to-door surveys has reached a critical mass, enabling the transition to card-based access.

Each digital health card will be linked to an individual’s Ayushman Bharat Health Account (ABHA) and serve as a secure, portable key to their lifelong health record. Citizens will be able to access medical history, prescriptions, lab and radiology reports, vaccination certificates, treatment summaries, and chronic disease management plans through a mobile app or QR code at any healthcare providerΒ  public or privateΒ  across the state. The cards will facilitate real-time updates during consultations, e-prescriptions, teleconsultations, and seamless referrals, reducing duplication of tests, medication errors, and delays in care continuity.

The government has prioritized privacy and security in line with the Digital Personal Data Protection Act (DPDP), implementing end-to-end encryption, consent-based data sharing, and strict access controls. Vulnerable groups, including rural residents, tribal communities, and low-income families, will receive special outreach through ASHA workers, mobile health vans, and community camps to ensure inclusion and literacy on using the cards. Officials noted that the project has already digitized records for millions through existing ABDM adoption in Telangana, and the cards will build on this foundation to create a truly citizen-centric digital health ecosystem.

The rollout will begin in phases, starting with urban centres like Hyderabad, Warangal, and Nizamabad, followed by district headquarters and rural mandals. The state is collaborating with technology partners to develop user-friendly interfaces in Telugu and English, integrate with major hospitals and diagnostic chains, and provide offline access options for low-connectivity areas. Health Minister T. Harish Rao emphasized that the digital health cards will not only improve individual care but also enable better public health surveillance, disease tracking, and policy planning through anonymized aggregate data.

This move positions Telangana as a frontrunner among Indian states in digital health infrastructure, complementing national efforts under ABDM while addressing local challenges such as high chronic disease prevalence and uneven access to specialists. The government expects full issuance to cover the entire population within the next 12–18 months, with ongoing updates to keep records current and relevant.
