The Indian government has integrated the Poshan Tracker app with Ayushman Bharat Health Account (ABHA) under the Ayushman Bharat Digital Mission (ABDM), enabling a single, secure digital profile that combines nutrition, immunization, and health data for pregnant women, lactating mothers, and children.

Poshan Tracker, originally developed to monitor nutrition services delivered through Anganwadi centres under the Integrated Child Development Services scheme, now connects seamlessly with the ABHA framework, enabling authorized healthcare providers to access a complete view of a beneficiary’s health journey.
For young children, the integrated system brings together key details such as birth records, immunization history sourced from the U-WIN platform, and regular growth monitoring data captured via the Poshan Tracker. All of this becomes accessible through a single ABHA-linked account. Similarly, for pregnant and lactating women, the platform unifies records related to nutritional support, antenatal check-ups, and delivery information. This holistic approach minimizes the need for repeated data entry and ensures better visibility across different stages of development and care.
The integration enhances real-time data flow between nutrition and health departments, empowering Anganwadi workers, clinicians, and programme administrators to track progress more effectively. Frontline health workers can now identify gaps quickly, ensure timely interventions, and provide more coordinated support.
