Baltimore based b.well Connected Health has formed strategic partnerships with health insurer Humana, behavior change platform Noom, and cardiometabolic solutions provider Welldoc to improve secure access to patient health records. The collaborations support the Centers for Medicare & Medicaid Services (CMS) Health Technology Ecosystem initiative, enabling users to aggregate and share data across providers, plans, pharmacies, and digital apps for more coordinated and personalized care.
Glimpse:
Announced on April 9–10, 2026, the partnerships leverage b.well’s national health data network which connects nearly 2.4 million providers and 350 health plans along with its 13-step data refinery process to normalize and enrich fragmented information into complete longitudinal records. Humana will use real time data for smoother claims processing, Welldoc will integrate AI-powered insights for chronic disease management, and Noom will launch “Health Record Connect” for Medicare members, offering the first 30 days of its Noom Weight program free to new users.
b.well Connected Health, a leading digital health platform based in Baltimore, has announced multiple partnerships aimed at breaking down data silos and improving how individuals access and use their health information. The company is working closely with Humana, Noom, and Welldoc to expand secure data exchange in line with the Centers for Medicare & Medicaid Services (CMS) Health Technology Ecosystem initiative. This effort helps patients securely connect their medical records across different providers, health plans, pharmacies, and digital health applications, ultimately supporting better coordinated and more personalized care.
Through the partnership with Humana, members can now link data from various sources in one place. Humana gains the ability to access real-time information during claims processing and respond more efficiently to requests from providers and other payers. This reduces administrative burdens and enhances care coordination while aligning with broader goals of reducing waste in the healthcare system.
Welldoc, known for its AI-enabled solutions focused on cardiometabolic conditions such as diabetes and obesity, will integrate patient authorized health data from b.well’s network. This connection provides Welldoc users with more complete, longitudinal records, allowing the platform’s AI to deliver timely, personalized coaching and clinical decision support exactly when patients and clinicians need it most.
In collaboration with Noom, b.well is introducing a new feature called Health Record Connect, specifically for Medicare beneficiaries. Users can link their medical records to the Noom platform in just minutes, enabling tailored behavior change interventions based on diagnoses, medications, and lab results. New Medicare members joining Noom will also receive the first 30 days of the flagship Noom Weight program at no cost.
b.well’s national health data network serves as the backbone of these integrations. It employs a sophisticated 13-step data refinery process to normalize, deduplicate, and enrich information from multiple sources, creating a unified and accurate health profile for each user. These partnerships represent a practical step toward true interoperability, giving individuals greater control over their data while helping digital health tools and insurers deliver more effective, data driven support.
“One of the biggest barriers to truly personalized healthcare is data silos; patient health records aren’t easily shared between providers, health systems, and digital platforms. With this integration, we’re changing that. A Medicare member can now connect their health records to Noom in minutes.”
