Zynix AI Nears One Million Patients with Care Execution Agents Transforming Value Based Care

Florida based Zynix AI has rapidly scaled its AI-powered Care Execution Agents to support nearly one million patients across multiple value based care networks. The intelligent agents automate essential care tasks, enabling providers to close care gaps, improve coordination, and reduce administrative workload in accountable care models.

Announced on April 15, 2026, the platform is now live with partners including Palm Beach ACO, Eternal Health, NHS-affiliated programs, Innovacare, and Privia Health. By integrating data from electronic medical records, claims, and clinical systems, the agents manage post discharge follow ups, annual wellness visits, chronic disease monitoring, and proactive care gap closure. According to the company’s press release, Zynix AI has also surpassed a valuation of USD 100 million and is expanding its team and investor network.

Zynix AI has achieved a major milestone in the value based care landscape by deploying its Care Execution Agents across nearly one million patients. The healthtech company’s AI-driven platform is designed to help accountable care organizations and health systems deliver more proactive, coordinated, and efficient care without adding to clinician workload.

The Care Execution Agents operate by aggregating data from multiple sources, including electronic health records, claims data, and clinical documentation. Using this unified data layer, the agents automatically execute key care activities such as post discharge patient follow ups, scheduling preventive wellness visits, monitoring chronic conditions like diabetes and hypertension, and identifying and closing gaps in recommended care.

This large scale deployment includes collaborations with several prominent healthcare organizations, including Palm Beach ACO, Eternal Health, NHS-affiliated programs, Innovacare, and Privia Health. The platform is designed to integrate seamlessly into existing clinical workflows, allowing healthcare providers to focus more on patient care while AI handles routine coordination and administrative tasks.

In addition to this operational expansion, Zynix AI has crossed a valuation of over USD 100 million, reflecting growing investor confidence in AI-driven healthcare execution models. The company is actively hiring across engineering, AI, and product teams while engaging with strategic investors to support its next phase of growth.
