eClinicalWorks has launched a new CCM (Chronic Care Management) Specialist Service to improve care for chronic patients while reducing workload on healthcare staff through integrated, proactive support.

The solution helps healthcare providers expand care without hiring additional staff while improving patient engagement and outcomes.

eClinicalWorks has launched the healow Chronic Care Management (CCM) Specialist Service, a new offering integrated into its electronic health record (EHR) platform to enhance long-term care for patients with chronic diseases.

The service embeds trained CCM specialists directly into healthcare providers’ workflows, enabling structured monthly outreach to patients suffering from conditions such as diabetes, hypertension, and cardiovascular diseases. This ensures continuous monitoring, better adherence to treatment plans, and improved care coordination between visits.

One of the key challenges in chronic care management is the lack of consistent follow up due to staffing limitations. The new CCM Specialist Service addresses this gap by assigning dedicated professionals who conduct regular patient check ins, review care plans, and document interactions in real time within the EHR system.

Early adopters have reported significant operational improvements. For example, healthcare practices using the service have been able to expand chronic care programs without hiring additional staff, while also reducing patient call volumes and improving engagement.
