Bihar Leads India in ABHA Scan & Share OPD Registrations Over 91% Paperless in Government Hospitals

Bihar has emerged as India’s top performer in digital OPD registrations under the Ayushman Bharat Digital Mission (ABDM), with over 91% of patients in government hospitals using the ABHA-based Scan & Share QR code system. This dramatic shift from just 4% earlier in 2025 has generated nearly 3 crore tokens nationally, positioning Bihar ahead of Uttar Pradesh and Andhra Pradesh in adoption rate and efficiency gains.

Bihar’s integration of the Mukhyamantri Digital Health Yojana (MDHY) with ABDM has enabled all 592 government facilities to offer QR-based registrations, slashing wait times, enabling real-time monitoring, and creating ABHA IDs on-the-spot. Nationally, over 11.41 crore OPD tokens have been issued since launch, with Bihar contributing 2.95 crore driving paperless workflows, reduced queues, and better resource allocation in public healthcare.

Bihar has set a national benchmark in digital healthcare by achieving the highest adoption of ABHA Scan & Share for outpatient department (OPD) registrations, with over 91% of patients in government hospitals now using the QR-code system. This marks a remarkable turnaround from a mere 4% adoption rate earlier in the year, propelled by seamless integration with the state’s Mukhyamantri Digital Health Yojana (MDHY).

The ABHA-based Scan & Share service, launched under the Ayushman Bharat Digital Mission (ABDM), allows patients to scan a QR code at registration counters, instantly generate or link their Ayushman Bharat Health Account (ABHA), and receive digital tokens eliminating long queues and paperwork. In Bihar, this has transformed patient flow average registration times have dropped significantly, first-patient arrivals are earlier, and facilities report smoother operations.

All 592 government health facilities across the state now support digital OPD, with dedicated staff assisting patients particularly the elderly and rural visitors in creating ABHA IDs on-site. This has not only boosted convenience but also enabled electronic prescriptions, lab reports, and pharmacy dispensing, paving the way for fully paperless care.

National Health Authority (NHA) data underscores Bihar’s leadership: Of the 11.41 crore total OPD tokens generated since ABDM’s inception, Bihar accounts for 2.95 crore, surpassing Uttar Pradesh (2.26 crore) and Andhra Pradesh. This high penetration rate (over 91%) reflects strong state-level commitment, including real-time dashboards for monitoring and incentives under ABDM’s Digital Health Incentive Scheme.

The success has broader implications reduced administrative burdens allow staff to focus on care, while digital records enhance continuity and fraud prevention. Bihar’s model combining national ABDM infrastructure with local schemes like MDHY offers a replicable blueprint for other states aiming to bridge urban-rural healthcare divides.

As India pushes toward a unified digital health ecosystem, Bihar’s strides highlight how targeted implementation can deliver equitable, efficient public services at scale.
